If you pre-register, then you will only need to confirm your personal information and show your photo ID and insurance card when you arrive at the hospital. Step OneComplete the pre-admission form. This secure, online form will be automatically submitted to OB Registration. After completing the form, you may proceed to step two. Before you start filling out the form, please have:
Step TwoDownload the OB Registration Packet. Please review the content of this packet for information regarding your hospital stay, including forms and a review of your patient rights and hospital services. A checklist is included to help you ensure that you complete all forms as indicated. After reading and signing all forms, please return via mail or in person to OB Registration. Don't forget to include a copy of the front and back of your insurance card(s). It is also a good idea to make copies of your signed forms to file before you return the originals. En Espanol
